Reporting an Absence (or Upcoming Event that will lead to an Absence)

 

WHEN A STUDENT IS SICK / ABSENT FROM SCHOOL: 

  • Parent/Guardian is to complete the following Google Form: https://forms.gle/GRbEcXrpoD4LBZMx7
  • When student returns to school they are to bring a parent/guardian note regarding their absence to school. Student is to show each teacher the note regarding their absence(s) and have their teachers sign the note. After note has been shown to all teachers, student is to deliver note to ROOM 119 (General Office). The student's absence note will be kept on file with the school.
  • All absence notes should have the Student's First and Last name, Grade Level and OSIS (Student ID) Number printed clearly.

ABSENT FROM SCHOOL DUE TO ATTENDING COLLEGE ADMISSIONS EVENT:

  • Student / Parent or Guardian / Care Taker communicates with their teachers and guidance counselor ahead of the scheduled College Admissions date to make them aware that the student will not be attending classes that day.
  • Parent/Guardian is to complete the same Google Form linked for when a student is sick / absent.
  • Student includes documentation to validate the college event they are attending.
  • Student brings note/confirmation from attending the College event, and has all teachers sign it and return the completed note to ROOM 119 (General Office).
  • All absence notes should have the Student's First and Last name, Grade Level and OSIS (Student ID) Number printed clearly.

NO EXCUSED ABSENCE (Except for a NYC DOE Trip)

  • There is no note that can excuse an absence. This documentation is kept on record to accurately calculate students attendance who are below 90%.
 
ABSENCE DUE TO STUDENT TESTING POSITIVE FOR COVID:
      • Parent is to send an email to Assistant Principal Alexis Kirschbaum at [email protected]
      • Mrs. Kirschbaum will respond to email with directions concerning student's return date to school.
      • Parent/Guardian is to complete the following Google Form: https://forms.gle/GRbEcXrpoD4LBZMx7
  • When student returns to school they are to bring a parent/guardian note regarding their absence to school. Student is to show each teacher the note regarding their absence(s) and have their teachers sign the note. *** Please be advised: A parent/guardian does not need to call the school regarding a student absence, instead please follow the instructions above.
